Public Information Request
The City of Waverly provides the following ways for you to submit your public information request.

Hand Delivery or Mail
You may request a form from the City Hall and hand deliver or mail it.
Written Form
You may submit a request to the FOIA Officer or the Chief of Police in any written form, including by electronic mail, fax, or letter.
Good to Know
Please note that for a public information request to be valid, it must be submitted in writing to the City.
To reimburse the city for its cost for reproducing and certifying the records, you will be charged the following fee: $.15 per page for employee copied records (first (50) copies are free) and $1.00 per page for certification of records. In addition, for commercial requests the city charges $10.00 for each hour spent by personnel in searching and retrieving a requested record or reviewing the record for necessary redactions; however, no fees will be charged for the first 8 hours spent by personnel in searching for or retrieving a requested record.
